Renew Your License

You will be notified by mail 60 days prior to the expiration of your license:

1. Download, print and complete the QHLB’s renewal application.

2. Fax, mail or email us a completed renewal application
3. Remit $55 on-line by credit card using the IPX Services Credit Card Gateway. When you choose this option your credit card statement will show a $45 payment to IPX Services. Or remit $45 check or money order by mail to:
